Complete the application form by using a typewriter, printing in black ink, or using your computer to enter the information directly onto the form from the Adobe PDF document. Hand printing must be clearly legible. Applications should be completed properly using the following instructions:
- Complete each section in full.
- Application must be signed by the candidate AND employer or supervisor, if applicable.
- ONE letter of recommendation, no longer than two pages, is required. Additional recommendations will not be considered.
- Do not include other attacments such as resumes. Additional attachments will not be considered.
- A $25 non-refundable application fee must be included with the application.
- Return the original application and letter of recommendation along with SIX COPIES of both the application and recommendation letter by close of business on March 31.
- Attach a professional studio 5x7 portrait which will be used for the LeaP 2008 Class directory. Photos may be e-mailed if they are at least 300 dpi and in JPG or TIF formats. Passport style and photos printed on computer paper will not be accepted.
- Photographs will be removed from the application prior to being submitted to the Selection Committee. Photographs will not be returned.
Your application will be not be considered if the application is received late, if it is not complete, if the recommendation form or letter is not enclosed, or if the photo is missing.
